Dr. Melvin Malmstrom, Board Chair
Dr. Melvin Malmstrom has acted as SLDDS' Board Chair of Directors since 2004. He has served on the Board since 2002 and is currently the Chairman of SLDDS' Clinical Committee.

Dr. Malmstrom has his Bachelor of Science in Biology from the University of Utah and graduated with his Doctor of Dental Surgery from University of the Pacific, Arthur A. Dugoni School Dentistry. He practiced dentistry for 15 years in West Jordan, Utah and acted as President of the Salt Lake District Dental Society in 2002. Dr. Malmstrom now pursues his interests in traveling, mathematics, and languages.

Stephanie M. Jensen, Executive Director
Stephanie Jensen has been the Executive Director of Salt Lake Donated Dental Services since May of 2003. She was previously employed with the organization from February 2002 through May 2002 as the Front Office Coordinator and then on a volunteer basis from February 2003 through April 2003.

Ms. Jensen has her Bachelor of Arts in English as well as Art from Westminster College. Prior to working for SLDDS, Ms. Jensen managed a private dental practice (1992-1997; Pioneer Valley Dental), worked internationally with grassroots community projects in Central Asia (1999-2001; Peace Corps, Turkmenistan) and developed and implemented a local dental insurance company’s HIPAA compliance plan as required by the HIPAA Privacy Standards of 1996 (2002-2003; Dental Select).
